StayWell News from UCOP -- Earn your $100 StayWell incentive with UC Walks
Just in from UC Office of the President (UCOP), Staywell eligible employees may participate in UC Walks: Cal Walks@Work Day as an alternate follow-up program to earn the StayWell incentive award.
- Register for UC Walks: Cal Walks @Work, choosing to walk on your own or to walk with the organized walks to the Celebration Event.
- Record your participation in your StayWell PointsBank at the StayWell website after you walk on May 23.
While StayWell participants can complete the UC Walks: Cal Walks@Work event before taking their health assessment, members still must complete the health assessment by the June 15, 2012 deadline in order to earn the incentive award. The incentive award is a $100 gift certificate for an eligible employee and a $50 gift certificate for an eligible spouse/domestic partner.
Employees and their spouses/domestic partners must be enrolled in an eligible UC medical plan on or before January 1, 2012, to be eligible for the incentive. Kaiser members and members of certain bargaining units are not eligible for the StayWell program.
StayWell Health Assessment
Make your health a priority this year starting with the 2012 StayWell Health Assessment (HA). Faculty and staff eligible for the StayWell program can earn a $100 gift certificate ($50 gift certificate for eligible spouses or domestic partners). Kaiser Permanente members, postdocs and members of certain bargaininig units are not eligible for the StayWell program. See the eligibility list.
- You can earn the incentive by completing both steps to earn 100 points by the following deadlines:
1. Complete the StayWell Health Assessment (HA)
by June 15, 2012 and earn 50 points.
2. Complete the StayWell follow-up program

StayWell Two-Step Program Details for 2012
Once your have completed the Health Assessment (step one to earn 50 points), you will need to complete the follow-up program (step two to earn another 50 points) to be eligible to receive your incentive award. Your HA results will determine the appropriate follow-up action based upon your health risks. You will be guided to either enroll in the NextSteps Wellness Coaching or the Time Out for Health program. |

StayWell NextSteps Wellness Coaching StayWell NextSteps coaching is a behavior-based program designed to help you set and meet your goals. If your HA results indicate that you are eligible for NextSteps wellness coaching program, there are three ways to participate: phone, mail or online. All the methods can take 3-6 months to complete so it is advised to register for Next Steps by July 31, 2012.
If you are not eligible for a NextSteps program, you will need to complete the Time Out for Health online campaign, a 6-week program focusing on nutrition, physical activity, stress management and the importance of hydration. You must complete your follow-up program no later than December 15, 2012 to earn 50 points toward your incentive. To access Time Out for Health, click on the My Invitations tabe and register.

Once you complete the Health Assessment and follow-up program earning a total of 100 points, you can expect an email (or a letter if you didn't provide an email) within about a week from StayWell Health Management with a link to the Hallmark Business Solutions website where you can choose your $100 Premiere Choice gift certificate from over 300 vendors, including UCB Recreational Sports. UCB Recreational Sports (RSF) Bonus Reward When selecting your gift certificate from Hallmark Insights, if you select UC Berkeley Recreational Sports and spend it on RSF non-membership services, you can receive an extra $70 toward an annual membership or two additional months for existing members. Kaiser members can show the email confirmation of completing the Health Assessment to receive the same bonus reward. Go to Recreational Sports page for more details. |

Your Privacy and Confidentiality When completing the HA, you will have the option to decide whether
or not to share your personal information with your Health Plan (Blue Cross, HealthNet, Kaiser). You
can exercise this option by indicating your preference at the end of
the assessment. |
