Know Your Numbers
Health Screening

Know Your Numbers is a fasting health screening offered by Health*Matters. The screening measures total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, body mass index (BMI), waist circumference and body fat. Your confidential results, seen only by Health*Matters staff, are provided immediately.

Knowing your numbers is one of the best ways to evaluate your health and to provide motivation for managing your health. Even if you feel great and look healthy, you may not know what's going on inside. Many diseases are "silent" and affect your health long before you feel anything wrong. Knowing your numbers can help identify your risks and including your numbers in the Health Assessment through StayWell or Kaiser gives you a more complete picture of your health status.

To enroll, log in to Blu, click on UCB Learning Center at the bottom of the Self Service Section and type "Know Your Numbers" into the search bar. The cost of the screening is $10.

By completing your confidential Health Assessment, you can gain insight on:

  • Health risks you may currently have but don't know about.
  • Information about topics such as weight management, tobacco cessation and cholesterol.
  • Tools to help you make positive health behavior changes.

Please note: To get the most complete picture of your health risks, it is best to know your blood pressure, cholesterol level and glucose level. Know Your Numbers Health Screening can provide you with this information; however, you can still complete the Health Assessment without these values and your results will be based on the information you provide.

Kaiser Permanente Health Works

UC Living Well, in partnership with Kaiser Permanente's HealthWorks, encourages Kaiser members to complete the Total Health Assessment.

Please note: To get the most comprehensive results from your Health Assessment, it is recommended (though not required) to include your personal biometric measurements and health screening results for height, weight, waist circumference, total cholesterol, HDL and LDL cholesterol, blood pressure and blood sugar. Know Your Numbers Health Screening can provide you with this information; however, you can still complete the Health Assessment without these values and your results will be based on the information you provide.

Your Privacy and Confidentiality

When completing the HA, you will have the option to decide whether or not to share your personal information with your Health Plan. You can exercise this option by indicating your preference at the end of the assessment.

UC will never receive personally identifiable information about you from StayWell or from Kaiser. UC will receive only de-identified summary information for the purposes of reporting and wellness program planning.
