What is a health assessment and how can it help me?

A Health Assessment (HA) is a confidential questionnaire that helps you examine the lifestyle choices you make every day and how those choices may be affecting your overall health.  When you complete the HA, you are provided with a customized action plan to help you succeed in making healthier lifestyle choices.  Your HA, available through StayWell or Kaiser, helps you:

• Learn about health risks you may have but aren't aware of
• Access tools and resources to help you make positive health behavior changes
• Gain information about topics such as weight management, nutrition, stress management, and smoking cessation.

Your privacy and confidentiality

When completing the HA, you will have the option to decide whether or not to share your personal information with your Health Plan (BlueCross, HealthNet, Kaiser). You can exercise this option by indicating your preference at the end of the assessment.

How we use the Health Assessment

The University of California (UC) and UC Berkeley will never receive personally identifiable information about you from StayWell or from Kaiser. UC will receive only aggregated reports about the program as a whole. This summary information will be used by UC for the purposes of wellness program planning.
