Counseling Services:
Academic Adjustments Related to Medical Conditions

University Health Services (UHS) provides treatment for illness and injury, and education to promote health. If your medical condition interferes with your ability to maintain your current course load, UHS can provide a Verification /Evaluation of Medical Care form as supporting documentation for an academic adjustment to:

  • Drop one or more classes
  • Bring your study list below minimum units
  • Take an incomplete
  • Change your grade option to Pass/Not Pass
  • In extreme cases, we will assist you in temporary withdrawal from school

UHS clinicians are unable to write notes and/or give excuses for absences, missed exams or assignments turned in late. We recommend that in these cases you discuss the situation directly with your professor(s). If you need evidence of having been seen at UHS, you can request a Confirmation of Medical Visit form by going to the Medical Records Department, Room 3220, Tang Center, 2222 Bancroft Way. Your confirmation will be completed the next working day.

For academic adjustments related to emotional or psychological issues, please speak directly with your counselor/psychiatrist in Counseling and Psychological Services. If you are not in treatment at UHS, we are unable to document your situation.

Course drop, Reduced Study List, Incomplete Grade, Grade Option Change
For students with serious medical conditions, the UHS can provide supporting documentation for a reduced study load, an incomplete grade, or a change to Pass/Not Pass grade option. We use UHS medical records or letters and records from private physicians to evaluate the severity of your illness and/or disability without revealing the diagnosis or releasing any confidential information. Our evaluation acts as supporting documentation for the Dean of your College; the Dean or the professor makes the actual decision. Drop petitions are available at 113 Campbell Hall, 642-1483.

If you would like to pursue this type of academic adjustment, go to Medical Records, Room 3220, Tang Center, and complete a Request for Verification/Evaluation of Medical Care form. If you received care outside UHS, we will need documentation of such care. When all the needed documents are received by UHS the information is evaluated and a Verification/Evaluation of Medical Care form is completed within three working days.

If your medical condition involves sexual assault, sexual harassment or substance abuse, please speak directly to a confidential counselor in the Social Services Unit regarding any of the above mentioned academic adjustments, by calling 642-6074.

Medical Withdrawal from the University
When your medical condition is so serious that you cannot continue to study, attend class or work, discuss your situation with your College Advisor. If you determine that a major academic change is warranted, contact Social Services at 642-6074 or go to Room 2280 Tang Center. Social Services staff will provide information about the criteria used to make recommendations for temporary withdrawals based on medical grounds, and how to provide documentation for such an assessment.
