Important SHIP Changes for Plan Year 2007-08

Temporary ID Cards available from Blue Cross

New policy on authorization of emergency services effective November 1, 2007:

Retroactive authorization is required for visits to an emergency room or urgent care center prior to November 1, 2007. You must contact the Student Health Insurance Office at (510) 642-5700 to ensure that your services will be covered appropriately.

No authorization from the Student Health Insurance Office is required for visits to an emergency room or urgent care center on or after November 1, 2007. Please provide the facility with your Blue Cross card. Claims submitted to Blue Cross will be processed according to the SHIP benefits.

All follow-up care must be authorized in advance by the Student Health Insurance Office.

Beginning August 15, 2007:

For UC Berkeley Students:

• The SHIP premiums for the 2007-08 academic year are $610 per semester for undergraduates and $805 per semester for international undergrads and graduate students at Berkeley.
• Podiatric services (non-surgical) will be covered at 80% of charge up to a maximum of $125 per plan year.

For Mills College Students:

• SHIP premiums, including Campus Care, are $1,146 per semester for undergraduates and $1,237 per semester for graduate students.
• Podiatric services (non-surgical) will be covered at 80% of charge up to a maximum of $125 per plan year.
