KAISER Total Health Assessment

UC works in partnership with Kaiser Permanente's HealthWorks Program to bring Kaiser members web-based tools designed to empower members to take charge of their health. Assessing your health is the first step in taking charge and planning positive lifestyle changes. Kaiser members completing the confidential Health Assessment can:

• Become familiar with health risks you may currently have but don't know about
• Gain information about topics such as weight management, nutrition and hypertension
• Access tools to help you make positive health behavior changes

Start with the Total Health Assessment
1. Go to: Total Health Assessment with Succeed
2. Click on Health Media Succeed in the middle of the page and take your health assessment.
3. Sign in with your Kaiser User ID and password.

UCB Recreational Sports (RSF) Bonus Reward

Employees enrolled in the Kaiser Permanente health plan who complelte the Kaiser Health Assessment can get $70 off the cost of an annual Rec Sports membership. Current members will get a free two month extension of annual membership. Print out your health assessment confirmation email and bring it to the RSF cashier, along with a valid form of ID, where you will receive $70 off the cost of an annual Rec Sports membership.  Go to Rec Sports page for more details.

When completing the HA, you will have the option to decide whether or not to share your personal information with your Health Plan. You can exercise this option by indicating your preference at the end of the assessment.

UC will never receive personally identifiable information about you from StayWell or from Kaiser. UC will receive only de-identified summary information for the purposes of reporting and wellness program planning.
