Health Assessment

UC Living Well, in partnership with Staywell, will reward
eligible faculty and staff and their eligible dependents a $75
gift card (choice of 350 merchants) for completing the confidential
Health Assessment. You will receive a customized health improvement
plan and access to online programs, wellness coaches if you
have health risks, and more. Also see:
Recreational Sports Added Incentive

Total Health Assessment
UC Living Well, in partnership with Kaiser Permanente, has
announced the new Healthy Lifestyle wellness initiative in 2008.
Kaiser members are encouraged to start with the Total Health
Assessment. Kaiser's new reward program includes a quarterly
prize drawing for those who complete their Health Assessment
and includes each quarter one $500 prize and six iPods. Also

Please note: To get the most comprehensive results from your
Health Assessment, it is recommended (though not required) to
include your personal biometric measurements and health screening
results for height, weight, waist circumference, total cholesterol,
HDL and LDL cholesterol, blood pressure and blood sugar.

UC Yourself Living Well workshop now on video!
Watch
the 30-minute video of this workshop.
The UC Yourself Living Well workshop, presented by John
Swartzberg, MD, FACP, Chair of the Editorial Board of the UC
Berkeley Wellness Letter and faculty member in the UC Berkeley
School of Public Health, is designed to provide information
about today's health risks and the important role of diet, physical
activity and smoking cessation in managing health. Complete
the HRA and then watch this video to help you prioritize your
health improvement decisions.

Leading an Active Lifestyle at Work |
Health*Matters
- Health*Matters
Walking Group is convenient, free and a simple way to
fit 30-40 minutes of walking into your work day. Meets every
MWF, 12:10 pm at the Campanile. Led by Walking Leaders, the
walking group accommodates all levels of walkers, from beginning
and moderate-paced to fast-paced walkers. Participation is
open to all faculty and staff. Just show up wearing comfortable
walking shoes and sunscreen and be ready to walk! No registration
required.
- Step Up to Fitness
Workshop
Find out how you can fit fitness into your day by walking
10,000 steps wearing a pedomter. Also learn strength and stretching
exercises to round out your fitness routine. Accusplit Eagle
120 XL pedometers, $12 ($20 value). Scheduled for the fourth
Tuesday each month. This full workshop or the 15-minute pedometer
orientation are available for departments or work groups.

trial membership for faculty and staff, available anytime
in 2008 with one trial membership per person per year. Register

- Alive! A Lifestyle Intervention via Email
Alive! is a free email intervention supporting simple
and effective changes to your eating and exercise habits.
Complete a nutrition assessment prior to enrolling to receive
12 weekly email messages tailored to your lifestyle and preferences.
Created by SPH faculty member Gladys Block, PhD, and colleagues,
Alive! begins with a diet and physical activity questionnaire
you may access with or without enrolling in the program. The
questionnaire will give you INSTANT feedback about your intake
of saturated and trans fats, sugars, fruits and vegetables,
and about your physical activity. After you've completed the
questionnaire, you can choose to sign up for Alive!
