Thanks for visiting our survey site! You may have been directed here by request of your Health Worker, or simply because you would like to share your input with us as a resident of one of the houses, halls or communities that we serve. We appreciate you taking the time to tell us how we can do the best job providing health education and resources in your living center.

• Health Assessment Survey ("Survey 1") - helps us learn about your health-related experiences and beliefs
• Health Interest Survey ("Survey 2") - helps us learn about the types of health programming and information you want to see
  
  

Feedback and Suggestions
We welcome any feedback, compliments, complaints or suggestions about the Health Worker in your house, hall or community. You are welcome to do any of the following to share your input:

• Contact your Health Worker directly via stopping by, making an appointment, leaving a note, or sending an email
• Request to speak to his/her student Coordinator who can ensure the Health Worker receives appropriate praise or performance counseling
• Speak with one of the leaders in your house or hall, such as an RA, manager, officer or other leader, who can pass on your compliment or concern to the Health Worker
• Share your comments with the Program Coordinator who is a professional staff member that oversees the entire Health Worker Program - you may call (510-643-9061), email ( ) or schedule an appointment (2010 Tang Center)
• Notify University Health Services by completing an anonymous comment form located at the bottom of each stairway of the Tang Center, or by emailing the UHS Director's Office (uhsadmin@berkeley.edu)
