DeltaCare Option (for non-SHIP members)
Note: Dental coverage is included as part of the Student Health Insurance
Plan (SHIP). All UC Berkeley students enrolled in SHIP automatically
receive benefits covering preventive and general restorative dental

| What is DeltaCare? |
For students who waive SHIP, University Health Services provides
information on a dental insurance program called DeltaCare. DeltaCare
provides comprehensive coverage for dental services with an emphasis
on preventive care. Covered services include check-ups, cleanings,
x-rays, fillings, crowns, oral surgery, and periodontics. DeltaCare
is provided by Delta Dental Plan of California through an affiliate,
PMI.
This voluntary dental plan is available to all Cal students,
affiliates of UC Berkeley (i.e., Mills), and dependents. DeltaCare
is optional and not managed by the Student Health Insurance Office.

DeltaCare provides the following features:
- Only $97 for a full year of coverage
- No claim forms
- No deductible
- Low co-payments (as little as $5 for a visit)
- Emergency care available 24 hours a day, 7 days a week
- Specialty care available

| How DeltaCare works |
To use DeltaCare, you simply choose a dentist from
the network of PMI dentists and make an appointment. You pay only
a predetermined co-payment for any covered services you receive.
Currently, there are two network offices in Berkeley and 10 in Oakland
from which to choose. |
| About DeltaCare dentists |
Dentists are chosen with care. Before network membership is granted,
each dental office is screened to determine:
- Number of years the dentist has been in practice (minimum:
5 years)
- Level of care and services offered
- If the dentist offers 24-hour emergency services (required
for membership)
- Convenience of location

| How to enroll |
For an enrollment form, please call the Student Health
Insurance Office at 510/642-5700. You will receive a complete list
of covered procedures and the corresponding co-payments, along with
a full description of the limitations, exclusions, and administrative
policies of this DeltaCare program and a list of PMI dentists. |
