Spring 2012 WAIVER INFORMATION
The waiver period for Spring 2012 is now closed.
Waivers must be submitted each academic year. The next waiver deadline for Fall 2012 will be July 15, 2012. Check back this summer for more details.

WAIVER APPEAL FORM
If you have submitted your
online waiver and it was denied due to inaccurate or incomplete
information, you may complete a Waiver Appeal Form (pdf) and
resubmit as instructed. You must have already
completed the online waiver and received a denial to use this form. You
may also email the completed appeal form along with the required
documents to ship@uhs.berkeley.edu.
WAIVER REVERSAL
If you have waived SHIP enrollment for Spring 2012 and wish to reverse your waiver, you can fill out a Waiver Reversal form for Spring 2012 (PDF). If you do not have dual insurance (SHIP plus another health insurance plan) and SHIP will be your only insurance plan, you must show proof of termination of your former insurance plan.
