SHIP Waiver Appeal Status Update 9/19/14
Some families are still waiting for their SHIP waiver appeal decision. The new waiver criteria and auto-decision waiver application created confusion resulting in a surge of waiver appeals this fall. As a result, the SHIP office has been managing unprecedented numbers of waiver appeals and inquiries. Please accept our sincere apologies. We anticipate it may be a few more weeks until all cases have been cleared. Due to the unusual delay in reviewing insurance waiver appeals, students that have *only* SHIP premiums unpaid and who have a waiver appeal still pending review will not be charged a late fee on their CARS accounts - see FAQs. Students who have non-SHIP related costs still pending payment on their CARS accounts will be assessed the standard late fees. UHS will be working with UCOP, management, our SHIP student subcommittee and student government leaders to improve the process for next year. WAIVER APPEAL FORM
If you have submitted your online waiver and it was denied due to inaccurate or incomplete information, you may complete a Waiver Appeal form (PDF) and resubmit as instructed. You must have already completed the online waiver and received a denial to use this form. You may also email the completed appeal form along with the required documents to firstname.lastname@example.org.
WAIVER REVERSALIf you have waived SHIP enrollment for Fall 2014 and wish to reverse your waiver, you can fill out a Waiver Reversal form (PDF). If you do not have dual insurance (SHIP plus another health insurance plan) and SHIP will be your only insurance plan, you must show proof of termination of your former insurance plan.
