Medical Records
When you check-in for your first appointment at the Tang Center, you will receive a "Gold Card" imprinted with your name and medical record number. This Gold Card or medical record card will be needed whenever you check in for an appointment in the Tang Center. (no more Gold Card! Click here for details)

| Transfer medical records to the Tang Center |
When you arrive at Cal, you may wish to have your medical records transferred to the Tang Center from your previous health care provider. You can print a release form for transfers of records to UHS (pdf or word format) or pick up a copy of the form from Medical Records. Complete the form and mail it to your previous health care provider. |

| Obtain medical records for personal use | You may request a personal copy of your medical records. You
may print a form for release of records from UHS (pdf or word format) or pick up a copy of the form from Medical Records. The
form must be completed in its entirety. You may mail the

Please note that for records of more than 2 pages, there is a duplication fee of $10 for up to 40 pages, plus $10 for each additional 40-page increment. An additional fee applies for a professional review or preparation of a record summary. Medical Records staff will inform you of the total cost once the form is received. Payment must be received (by mail or in person) prior to release of your medical records. Depending on time of year, processing may take 7-10 business days. Please note that in order to maintain patient confidentiality, the UHS Medical Records department does not fax medical records. If your last UHS visit date was prior to 2010, your chart is filed at our offsite storage facility. Please allow up to 5 weeks for processing time. |
