| Hepatitis B | For those under 19 on the first day of classes, proof of Hepatitis B immunization is required as a condition of enrollment. Learn more about the Hepatitis B requirement or simply download the Hepatitis B statement form. |
| Immunizations | In addition to the standard pediatric immunization series and Hepatitis B immunization requirement, certain immunizations are recommended for college students. Update 6/28/10: Because of a Whooping Cough outbreak in California, pertussis vaccination is critical. Find more details here from CA Dept of Public Health. |
| Insurance | All students are required to have major medical insurance to complement health coverage provided by UHS Tang Center. Learn more about the Student Health Insurance Plan (SHIP). |
